Credentialing Authorization

This authorization explains the administrative authority you give GetPaneled to perform insurance credentialing, payer enrollment, CAQH setup or maintenance, and related workflows for the services you request.

1. Credentialing Authorization

By submitting onboarding information, signing this authorization, checking an acceptance box, purchasing services, or otherwise approving GetPaneled to begin work, you authorize GetPaneled and its team members, contractors, service providers, and authorized representatives to assist with insurance credentialing, payer enrollment, CAQH setup or maintenance, and related administrative workflows on your behalf.

This authorization applies to the providers, practices, entities, payers, states, and services you request or approve.

2. Authorized Activities

You authorize GetPaneled to collect, organize, review, prepare, submit, update, and maintain credentialing-related information, documents, applications, rosters, payer forms, directory records, portal submissions, recredentialing materials, and related communications as reasonably needed to provide the services you request.

You authorize GetPaneled to communicate with insurance payers, credentialing departments, CAQH, payer portals, clearinghouses, enrollment platforms, government enrollment systems where applicable, and related credentialing entities to submit materials, request status updates, respond to correction requests, receive notices, and perform administrative follow-up.

3. CAQH Access and Maintenance

If you provide GetPaneled with CAQH access credentials or other CAQH account-access information, you authorize GetPaneled to access your CAQH account for the limited purpose of providing the services you requested.

This may include reviewing your profile, entering or updating information, uploading documents, authorizing payer access, correcting errors, maintaining your profile, tracking document or attestation deadlines, and completing CAQH re-attestation based on the information available to GetPaneled.

You understand that CAQH information may be used by payers and other organizations for credentialing, directory, enrollment, and related purposes. You remain responsible for the truthfulness, completeness, and accuracy of your CAQH profile and credentialing information.

Unless you notify GetPaneled of changes, you authorize GetPaneled to rely on the information, documents, answers, and approvals you have provided to complete CAQH maintenance and re-attestation.

4. Provider Updates

You agree to promptly notify GetPaneled of any changes to your license, malpractice coverage, practice address, contact information, employment, ownership, professional history, disclosures, sanctions, payer participation, tax details, or other credentialing information.

5. Signatures and Representative Authority

You authorize GetPaneled to use your typed name, electronic signature, uploaded signature, onboarding answers, written approvals, and provider-supplied information to complete credentialing and enrollment documents when the submission is based on information you provided or approved.

You also authorize GetPaneled to sign or submit as your authorized representative where a payer, portal, or credentialing form allows submission by an authorized representative.

This authorization does not allow GetPaneled to sign payer contracts, settlement agreements, legal documents, tax filings, loan or financial documents, employment agreements, or any document outside the credentialing, enrollment, CAQH, roster, directory, or related administrative workflow unless you separately approve that specific action in writing.

6. Electronic Records and Signatures

By checking an acceptance box, typing your name, signing electronically, submitting a form, or otherwise using an electronic workflow, you agree that electronic records and electronic signatures may be used for this authorization and related GetPaneled service documents.

You may request a paper copy or withdraw consent to electronic records by contacting hello@getpaneled.com. Withdrawal applies prospectively and may delay, limit, or prevent GetPaneled from providing services that require electronic authorization, submission, or records.

7. CAQH Maintenance Subscription

If you select ongoing CAQH maintenance, the maintenance add-on is billed monthly at the price shown at checkout for each covered provider. The subscription renews automatically each month until canceled.

Monthly billing is handled by Stripe. Unless a different billing date is shown in Stripe, each renewal is billed on the recurring billing date associated with your subscription.

You may cancel ongoing CAQH maintenance through your customer portal or by emailing hello@getpaneled.com. Cancellation stops future renewals at the end of the then-current monthly billing period. If you cancel within three days after a monthly renewal charge, you may request a refund for that renewal month by contacting hello@getpaneled.com.

8. Provider Responsibility

You represent that all information, documents, signatures, credentials, and approvals you provide to GetPaneled are accurate, current, complete, and authorized.

You understand that inaccurate, incomplete, outdated, or delayed information may cause payer delays, denials, rejections, incorrect records, or other issues outside GetPaneled's control.

9. Revocation

You may revoke this authorization prospectively by notifying GetPaneled in writing at hello@getpaneled.com. Revocation will not affect actions already taken in reliance on this authorization.

You understand that revoking authorization may delay, limit, or prevent GetPaneled from completing the requested services.
